Cleveland, OH – September 15, 2020: Family Care Path, LLC is pleased to announce its collaboration with Cordata Healthcare Innovations Cincinnati, OH.  This partnership will allow Family Care Path to assist Cordata to extend its platform’s features to include rich genetic and genomic risk stratification, client disease program growth through familial risk identification and whole-person risk profiling that considers future health risk outside of simply in-process disease care through the MyLegacy software tool.  

Complex and chronic diseases, like those managed on the Cordata platform, tend to run in families.  The typical disease care coordinator unfortunately does not have access to a patient’s family history, important genetic risk factors, or genetic testing results.  Unlike single-gene disorders, complex disorders may not have clear-cut patterns of inheritance, yet knowledge of mutations or family history can be a powerful tool in planning care (precision medicine). In tandem to the complex diseases, a majority of older adults (>60%) live with two or more chronic conditions.
