Their collaboration pairs automated genetic data gathering with care planning to save time, make care coordination activities more precise and streamline the full continuum of care.

Cleveland, OH – September 15, 2020: Family Care Path, LLC is pleased to announce its collaboration with Cordata Healthcare Innovations Cincinnati, OH.  This partnership will allow Family Care Path to assist Cordata to extend its platform’s features to include rich genetic and genomic risk stratification, client disease program growth through familial risk identification and whole-person risk profiling that considers future health risk outside of simply in-process disease care through the MyLegacy software tool.

Complex and chronic diseases, like those managed on the Cordata platform, tend to run in families.  The typical disease care coordinator unfortunately does not have access to a patient’s family history, important genetic risk factors, or genetic testing results.  Unlike single-gene disorders, complex disorders may not have clear-cut patterns of inheritance, yet knowledge of mutations or family history can be a powerful tool in planning care (precision medicine). In tandem to the complex diseases, a majority of older adults (>60%) live with two or more chronic conditions. Studies have long established that genes are contributors to chronic conditions such as cardiovascular disease, cancer, diabetes, obesity, rheumatoid arthritis (RA), Alzheimer’s disease (AD), and depression. Only recently, however, has it become possible to measure individual-level risks for these chronic diseases.  Bringing this information to bear in a care coordination setting is revolutionary.

Gathering comprehensive genetic and familial profiles on patients is typically a very manual process and a disparate one at that – where information is not considered, or even known, across the patient’s co-morbid conditions.  Normally, genetic assessment and testing is laser-focused on a single disease area like cancer or in the new area of cardiogenomics.  With the Family Care Path + Cordata solution, the impact on patient care coordination will be felt immediately with access to more genetic data, available precise care planning, increased pathway adherence, higher patient engagement and satisfaction, reduced costs, and a more holistic management of patients along their care continuum.

FamilyCarePath’s MyLegacy offers a cost-effective means to collect family health history compatible with the electronic medical record and the curated case on the Cordata platform.  The associated risks are conveyed for actionable health management to patients and their care teams. MyLegacy has helped to save valuable physician time while ensuring that genetically at-risk patients are identified – so that clinically appropriate measures are taken.  Adding the Family Care Path toolset to Cordata amps up the disease profile from standard histology, stage, acuity, risk and even social determinants – to considerations of the critical genetics driving disease occurrence and recurrence.

“We founded the company to introduce the web-based clinical decision support application MyLegacy, which uses proprietary algorithms based on practice guidelines developed by the Cleveland Clinic Genomic Medicine Institute.  Finding a partner like Cordata, allows us to bring this product to on-the-ground care teams and their patients – at lightning speed. Together, we are going to change what disease and person-centric care coordination looks like.” says Dave McKee, Family Care Path CEO

“At Cordata we are striving to provide a multi-faceted picture of each complex and chronic diseased patient.  This includes clinical, social, emotional – and genetic.  The more patient profile information our client disease Programs and their care teams have – the more precise their interventions can be.  Cordata’s mission is about moving the needle for the people affected by costly and life-altering diseases.  It takes a village to do this special kind of technology work – that has a greater good underneath it.  The team at Family Care Path shares our values and we are excited to see this work unfold.” says Gary Winzenread, Cordata’s President and CEO.
